Information may be used to respond to general inquiries, coordinate resource navigation, make appropriate referrals and improve community outreach. We do not sell personal information submitted to us.
Health information
This public website is not intended to be a HIPAA-secure patient portal. Before Mothers First Ohio accepts protected health information through an online referral process, the program should use an appropriate secure intake system and applicable privacy and security safeguards.
Third-party providers
General website form submissions are processed by a third-party form-delivery service so the request can be delivered to Mothers First Ohio by email. Do not use this form to submit protected health information, medical records, diagnoses, medication lists, insurance identification numbers, Social Security numbers, or other sensitive information. The form-delivery provider may temporarily process or retain submitted information according to its own practices. When a referral is made, the receiving provider or organization may also have its own privacy practices and may request information directly from you. Clinical care is provided by the independent provider, not by this website.
